ABSTRACT

AIM: To analyze the changes of serum homocysteine(Hcy), vitamin B12(VitB12)and folic acid in the serum of patients with diabetic retinopathy(DR), and to explore their significance in the occurrence and development of DR.METHODS: A case-control study was designed. A total of 95 patients with DR(DR group), 94 patients with diabetes mellitus(DM group)treated in endgcrinology department and 87 patients with age-related cataract(normal control group)from the ophthalmology department of Shenzhen People's Hospital between July 2021 and January 2022 were selected. Fasting venous blood was collected and serum was separated. The concentration of Hcy in serum was detected by enzyme linked immunosorbent assay(ELISA), and chemiluminescence immunoassay was used to detect the concentration of VitB12 and folic acid. Pearson linear correlation analysis was used to evaluate the correlation between Hcy and clinical parameters. Multivariate linear regression analysis was used to evaluate the main factors which affect Hcy level. Receiver operating characteristic(ROC)curve was designed to analyze the diagnostic value of serum Hcy, VitB12 and folic acid in DR.RESULTS: The concentration of serum Hcy in DR group was 16.52±3.54 μmol/L, which was significantly higher than that in DM group(10.86±3.47 μmol/L)and control group(6.84±1.39 μmol/L; all P<0.05); The concentration of VitB12 in the serum of the control group was 501.79±108.95 pmol/L, which was higher than that in DM group(478.57±57.85 pmol/L)and DR group(455.88±181.49 pmol/L), but the difference was not statistically significant(P=0.054); The concentration of folic acid in serum of control group was 10.31±2.43 nmol/L, which was higher than that of DM group(9.94±1.90 nmol/L)and DR group(7.27±2.79 nmol/L), and the difference between DR group and DM group was statistically significant(P<0.05); In DR group, Hcy expression was weakly positively correlated with triglyceride and low density lipoprotein(r=0.208, P=0.043; r=0.240, P=0.019). Multivariate linear regression showed that low density lipoprotein was an important factor which affect the expression of Hcy in DR patients. ROC curve shows that Hcy has important value in the diagnosis of DR.CONCLUSIONS: Hcy, VitB12 and folic acid are differentially expressed in DR group, DM group and normal control group. Hcy may be involved in the pathogenesis of DR, and it has important value in the diagnosis of DR. In addition, low density lipoprotein is also an important factor which affects the expression of Hcy.

ABSTRACT

Uveitis is a group of inflammation diseases involving the iris, ciliary body, choroid, vitreous body, retina and retinal vessels, which can lead to visual deterioration and even visual loss. The pathogenesis of uveitis is complex and diverse, including infection, autoimmunity, trauma, physical and chemical injury, immune genetic mechanism and so on. Recent studies have shown that the activation of complement system is one of the pathogenesis of uveitis. Various complement proteins, including CFH, CFB, CFI, MAC, CD59 and so on, regulate host tissue damage through a rigorous mechanism mediated by the complement system. And studies have found that those complement proteins are involved in the occurrence and development of uveitis at the gene level and biological function. In addition, complement components like C2, C3, C4 and C5 can affect the pathogenesis of uveitis in terms of copy number variation, gene polymorphism and the regulation of T-cell-mediated autoimmune response. Therefore, complement inhibition therapy and related gene therapy provide new ideas and targets for the treatment of uveitis.

ABSTRACT

Objective To propose a method for high-energy photon 3D decoding of PET detector to decode reactive crystal position and calculate γ ray depth in the scintillating crystal. Methods Light transmitting windows were set between the crystal arrays, visible photon groups were guided to distribute in the photoelectric sensor array during a single event, and sensor signal intensity was regulated to realize position and depth decoding of the high-energy photon.Monte Carlo method was used to compare the designs of different light transmitting windows to determine the effects of the crystal surface treatment mode on the results. Experiments were executed for further verification. Results The experimental results had consistency with simulation ones;the signal received through the reaction crystal was always stronger than that through the adjacent, which could decode the reaction crystals; the relations between the depth of interaction (DOI) and energy differences of the photo sensors were monotonic with the changes of DOI,and thus could be used to calculate the DOI;when using single window, the DOI vs.Diff.Energy curves had two segments which were not desired for DOI measurement;crystals with rough surfaces were more desired since the qualities of their DOI vs.Diff.Energy curves were better;the dual window method had the best DOI vs.Diff.Energy curves in term of linearity. Conclusion The method can realize 3D decoding of the discrete crystal array, which improves the energy resolution of the PET system. ABSTRACT

Objective To study the mechanical properties of PVP/PVA composite hydrogel and its interaction principle based on the molecular dynamics (MD) theory. Methods MD simulation was applied to investigate mechanical properties and radial distribution functions of PVP, PVA and their mixed system PVP/PVA. Results Compared with the pure PVP, mechanical properties of PVP/PVA were significantly improved, and not affected by temperature. The interaction between PVA and PVP was expected to occur through the interchain hydrogen bonding between the oxygen atom of PVP and the hydroxyl group of PVA. Conclusions MD method revealed the interaction mechanism of PVP/PVA hydrogel at molecular microscopic level and proved it better than pure PVP. Meanwhile, its mechanical properties were stable at different environment temperatures. These results provide a reliable theoretical research method for studying the fabrication process of hydrogel prosthesis in clinic and its mechanical properties.

ABSTRACT

Objective The purpose of this study was to understand the awareness, prevalence of diabetic retinopathy and treatment status of people aged over 50 and living in the rural areas of Shuangcheng city, Heilongjiang province, China. Methods Cluster sampling was used in randomly selected 5504 survey for ophthalmic clinical examination, in patients with diabetic retinopathy. A questionnaire in the state of knowledge about prevention and treatment was developed. Results Among the 5504 persons entering in the project, 5053 were examined on their eyes (91.8%). In this selected population, 56 persons (112 eyes) were diagnosed as diabetic retinopathy (1.108%), with 95% confidence interval (CI) as: 0.819% to 1.397%. Of 56 patients, 49 cases were non-proliferative diabetic retinopathy, accounting for 87.50% of the total number of patients with diabetic retinopathy;proliferative diabetic retinopathy 7 cases, accounting for 12.50% of the 112 eyes, 6.25% (7/112)having vitreous hemorrhage, 8.04% (9/112) having macular edema. For diabetic retinopathy prevalence rates, there was no significant difference in males and females. Between the per differential 10-year-old division, the difference was significant. Among the 60 to 69 group, a significantly higher prevalence rate was seen. Of the 112 eyes with diabetic retinopathy, 34 eyes(30.4%) were low vision [visual acuity <20/60 (0.3) to ≥ 20/400 (0.05) ]; 6 eyes (5.4%) were blind [visual acuity <20/400 (0.05) to NLP]. The rate in the patients with PDR and fasting blood glucose was above 11.1 mmol/L was higher than having NPDR and fasting blood glucose below 11.1 mmol/L. Having fasting blood glucose 11.1 mmol/L and above with the course over five years among patients with PDR, the proportion of fasting blood glucose was higher than those with less than 11.1 mmol/L and diabetic retinopathy duration of less than five years. Of 56 patients with diabetic retinopathy, 38 cases (67.9%) did not receive any treatment. Among 18 cases (32.1%) with insulin or oral drug therapy, regularly using insulin or other medication (14.3%), only 1 (1.8%) case was given the treatment for diabetic retinopathy. Results from our survey showed that patients with diabetic retinopathy had a poor understanding about prevention and treatment of the disease. Conclusion Long duration and high blood glucose in patients with diabetic retinopathy seemed to be the important risk factor. Early systematic drug use for prevention and blood glucose control was the key to prevent diabetic retinopathy. Patients with diabetic retinopathy in China had poor understanding about the prevention measures of the disease and lack of knowledge.

ABSTRACT

Diabetic retinopathy is one of the most common and serious microvascular complications of diabetes, and it is a very complex pathological process, displaying multi-factor, multi-stage characteristic in the outcome. Recent studies have shown that the pathogenesis of diabetic retinopathy is closely related to oxidative stress, and antioxidant therapy can effectively improve the condition. Heme oxygenase-1(HO-1)is a widely existing antioxidant defense enzyme, which can reduce the injury caused by oxidative stress and plays an important physiological role. Studies have indicated that in retina HO-1 can be highly induced by hyperglycemia. Moreover, the expression level of HO-1 by man-made changes can accelerate or postpone the diseases progression. That means HO-1 for the diagnosis and treatment of diabetic retinopathy has satisfactory applied foreground. In this article we would like to review the role of HO-1 in diabetic retinopathy.
